Below is the Mid America Modified Series Entry Form along with other forms you may need when entering a Mid America Modified Series event and a brief description for each:

Driver Entry Form – This form must accompany payment for advance entry and registration for any Mid America Modified Series event.

Driver License Application – This form must be filled out and approved by the Mid America Modified Series office if a driver is going to enjoy the benefits that include the possibility of higher event purses, increased tow money, discounted entry fees, contingency awards, points funds, increased media recognition and a personalized driver profile page on the Mid America Modified Series website. (Only needed once for the season)

Driver Registration – This form must be filled out and approved by the Mid America Modified Series office before a driver is allowed to participate in a Mid America Modified Series event. (Only needed once for the season)

MAMS Release – For insurance reasons this release is mandatory for anyone that enters the pit area at any Mid America Modified Event. It will always be supplied for your signature at the pit gate before your entry.

Minor Release Form – For insurance reasons this form must be filled out by both parents and notorized before any child under the age of 18 will be allowed in the pit area for any Mid America Modified Series event.
